  Getting Started/ New Patients

In Pennsylvania, it is recommended that you first see your doctor before beginning any therapy or exercise program. Please call us at 383-8700 to schedule your first one hour session within the next 2 days.  We will ask you for your name, a contact phone number, the name of your referring physician, your insurance carrier, and your diagnosis or place of injury.

We ask that you arrive a few minutes early and bring with you your insurance card and prescription from your doctor.  Before seeing your therapist, you will be asked to fill out a health history form, and sign a HIPAA and Medical Release Form.  These state that we cannot discuss your case with anyone other than you (or your guardian if appropriate) and your doctor, and that you authorize us to share documents with your doctor and insurance carrier, respectively.
